Chapter 244: Chapter 0228: A Tense Moment
The Affiliated Hospital of Tokyo University in Japan, boasts a history of 160 years.
This hospital, originating from the Edo era, has always occupied the top of the pyramid of Japanese medicine.
Even in the world of medicine, Tokyo University’s Affiliated Hospital holds an important place. Many new technologies originate from here, but it’s just that people don’t know about it.
The technology of arthroscopy for example, originated here. The USA and Europe once sent doctors for training here before taking the technology of arthroscopy back to America and Europe, where it slowly developed.
Tumor infrared fluorescent imaging technology also originated here. The world’s first case of liver cancer surgery using ICG fluorescence labelling was born in this hospital. Then, they developed the infrared 3D navigation system for liver cancer, pushing surgical operations of tumors towards precise digitization.
On Saturdays and Sundays, associate professors and professors usually rest, but today is an exception.
Fujiwara Masao, director of the Orthopedics department, leads the university attendees into the teaching room.
Inside the teaching room, doctors from three generations sit neatly arranged in a row according to their titles and seniority, from the front to the back, precise and formal, sitting straight up.
The hierarchy system of Japanese universities is also fully embodied in the affiliated hospital. A dean professor has absolute authority in his field without any interference.
Usually, case discussions take place every Tuesday and Thursday from 7:30am to 9:00am. However, today is Saturday and everyone has gathered together, without wasting any time. They have conducted supplementary case discussions for the entire week from 7:30am to 9:30am, for a full two hours.
In addition to preparing slides to assist with case explanations, the presenter also has to draw sketches on a whiteboard. The hand-drawn sketches are very exquisite because every doctor from his university days is required to spend one to two hours practicing drawing every night, which is an essential skill for doctors.
First, they draw according to anatomy books, then draw while referring to CT scans, and finally, during anatomy classes, they have to draw sketches referring to actual corpses. No margin of error is allowed when drawing any organ, blood vessel, or nerve. This is because, theoretically, doctors are not supposed to make mistakes.
With the help of these images, the presenter has to thoroughly explain the injured part, the injury mechanism, the degree of injury, the changes in pathological physiology and anatomical structure after injury, the impact on the surrounding organs, and the local and overall prognosis of the patient.
If there is even a slight loophole, the professors present would question it sternly. They use a tracking-style questioning technique, exposing all the knowledge related to the issue by asking a series of questions based on a single loophole.
For example, in the case of a pelvic fracture, the amount of bleeding, the total blood volume of the human body for children of different ages, adult men, women, and elderly people will all be queried.
The pathophysiological changes that occur when the human blood volume decreases by a certain percentage, the component configuration for blood transfusions, the combinations of crystalloid and colloid fluids for intravenous therapy will be discussed.
Under ischemic and hypoxic conditions, the pathophysiological responses of various organs of the human body and the medical principles hidden behind each test indicator is examined.
From clinical to anatomy, physiology, pathology, pathophysiology, tissue embryology, biomechanics and so on, knowledge loopholes are dug out with no place to hide.
After two solemn and intense hours of case discussion, they took a half an hour break. At ten o’clock, Tokyo time, or nine o’clock Chinese time, they began to watch the academic conference.
“Everyone, please concentrate. There is always something to learn from others. We must be good at learning from the strengths of others in order to become the strongest. I am relying on you all.”
Fujiwara Masao, in his fifties, has a thick and black short beard, bright eyes. He is also a sportsman. Sports help him maintain a robust body and abundant energy.
He loves the “Book of Five Rings” and always places a copy under his pillow. He also requires every student to become familiar with this book.
He believes that sincerity is the key to solving all difficulties in the world.
All morning, the teaching room was silent. Everyone concentrated and watched the academic conference.
“Sasaki, are you ready?” Fujiwara asked the Sasaki sitting next to him.
Sasaki is a bit younger than Takahashi. Although he doesn’t have the same amount of experience or academic standing as Takahashi, he is more talented than Takahashi and he has an abundant amount of energy. He is always in a state of excitement as if he is always in a state of hormone secretion. He doesn’t feel tired even after many surgeries.
Therefore, he is like an intelligent surgical machine. He never tires and never makes mistakes. Any complicated operation becomes accurate in his hands. Fujiwara appreciates him a lot.
“Sir, I’m ready! Ogawa team and Niiroi team are in place.” Sasaki slightly bowed with great respect.
“Very good. We must go all out for every surgery!” This is Fujiwara’s famous saying.
Ogawa Kumaide and Niiroi Hiro are both lecturers, equivalent to attending doctors, and subordinates of Sasaki. They are the first team responsible for this surgery demonstration. Then, the second and third teams are all in place.
“Good, unless necessary, you don’t have to go on stage. This is a demonstration of friendship, so don’t make it too embarrassing. Face is the most fragile thing in the world.” Fujiwara reminded again.
“Sir, I understand, I won’t go on stage unless necessary!” Sasaki couldn’t possibly go against Fujiwara’s wishes.
“What do you think of their five-section replantation?” asked Fujiwara Masao.
Sasaki had been watching the conference’s live broadcast just now: “Sir, this should just be a show. The origin of limb replantation is China, they have unique case resources, which can cultivate excellent microsurgeons. But for the five-section replantation to achieve such good results, it’s impossible. Even using our lab’s peripheral nerve recognition and growth-promoting technology, it’s hard to restore such an effect.”
“But, the facts are before us, they demonstrated the patient on stage, I can’t see any flaws.” Fujiwara respected the objective facts.
“Sir, pardon my frankness, your impression of China’s medical community might be stuck twenty years ago. Now, academic fraud is rampant there. Many so-called cutting-edge achievements eventually fall apart because they cannot be replicated, and some papers published in top international journals are found to be fraudulent and retracted. In their medical community, paper fraud is the norm.” Sasaki explained.
Fujiwara nodded, “Is that so? Confidence is a good quality, but conceit can block the whole view. We should still be cautious, Sasaki. You’d better pay attention to this case and use scientific means to verify the potential success of this case. Don’t rush to a conclusion.”
“Understood, I will keep an eye on it.” Sasaki promised.
It was already afternoon, everyone had just had lunch and rested briefly, and then began to watch the conference again. The speaker was a doctor from South Korea, Park Choon-ui.
At this time, a young doctor came in, walked quietly to Sasaki and whispered something in his ear.
After hearing the message, Sasaki made a gesture and the messenger doctor left the lecture hall.
“Sir, there was a car accident on the Palace Bridge Expressway, a multiple collision, one of the vehicles involved was an ambulance from a nearby hospital, our ambulance has already left.” Sasaki reported to Fujiwara.
“That’s terrible! Another accident has occurred in this area, just like a few years ago.” Fujiwara was very familiar with the area around Tokyo.
—
Takahashi received information from the Tokyo headquarters. The ambulance there had already left, and the demonstration could begin.
He spoke a few words with Fujiwara Miyuki, and then raised his hand to Tang Fei, who was not far away. Tang Fei immediately came over, and Takahashi said, “Our ambulance has already left. Can we switch to the surgical demonstration?”
Because the surgical demonstration was spontaneous, the two sides would not be synchronized, which was considered normal.
Tang Fei immediately reported to Director Tan, who then ordered, “After the lecture by our guest speaker, Doctor Park Chun-ui from South Korea, please pause for a while. Connect to Tokyo University Hospital on the screen and proceed to the surgical demonstration.”
The screen displayed a prominent caption: Upcoming surgical demonstration, Demonstration Hospital: Tokyo University Hospital.
The venue immediately became noisy. The most thrilling part of the conference finally arrived. The trauma emergency surgery demonstration by Tokyo University Hospital was about to begin.
Many people had lost interest in listening to the lecture. Some were drinking water, some stretching, some immediately went to the restroom, and some took off their glasses and started cleaning them.
When Doctor Park Chun-ui’s lecture ended, the repression in the venue instantly exploded with noise.
“Has the surgical demonstration started?”
“Yes, at Tokyo University Hospital.”
“I heard their ambulance has already left.” @@novelbin@@
“Quick, hurry, it’s starting!”
The screen flickered a few times and successfully switched to Tokyo.
The internet has connected the world. The slightest movements in the distant Tokyo were showing on the screen right in front of them.
Tokyo University Hospital, Trauma Emergency Center, shadows flickering, telephone ringing relentlessly.
The moment of tension has arrived!
What do you think?
